Procedure for Hospitalization

Doctor in charge of outpatient will decide hospitalization date and time and make reservation.

Please consult with Doctor in charge for possible reservation change, or cancellation for the reservation.

On the hospitalization day, please come to hospital General Reception Desk or Support Center on the designated time , or a bit earlier.

In case patient is unable to visit hospital at the designated hours, Please contact the hospital as soon as possible.

  • Medical Insurance Card
  • Recipient Certification Card (Eldery, Disabled, Inveterate Disease, etc,) "Applicant only"

Attendant

Essentially, attendant is not authorized at this hospital, but there may be a case where we consult with a family member.

Also, one can be an attendant if so desire. In this case, one needs to fill in a document.

Please ask to nurse station for requesting the attendant meal.

Medical Treatment consultation

Please ask to Medical Social worker (or Hospital ward nurse station) for any concerns such as medical expense, medical treatment or social reintegration.
