How to make reservation ro contact with the center

Please make a reservation by phone or by visiting direct.
※Please make a reservation two weeks prior to the desired date.
※Please understand we are unable to accept reservation due to the reservation congestion situation.

Reception Hour	10：00～16：30　 (excluding Saturday, Sunday and national holiday)
Medical Checkup	Monday through Saturday 8:00 AM to Noon

On medical checkup date

On the hospitalization day, please bring the following items on medical checkup day.

Bring in items	Medical Insurance Card Questionnaire Written Consent Form Granting Certificate for Medical Treatment

About Medical Checkup Result

We will mail you the checkup result within 3 to 4 weeks later.
You are able to receive the checkup result from the doctor in charge.
